• You can print out an application form at the Mississippi Division of Medicaid or Mississippi's Common Web Portal website.  Then, submit your completed application to the Office of Eligibility at the Division of Medicaid.
• Fax your application to 1-601-576-4164. (An origianl signature is not required on faxed applications.)
• Mail your application to 550 High Street, Suite 1000, Jackson, MS 39201 or the regional office that serves the county where you live.
